Description
The Newport News Sheriff's Office is in the process of implementing a Medication-Assisted Treatment (MAT) Program to help those opioid and alcohol dependent. MAT will enhance existing substance abuse treatment services within the jail and community. The recovery case manager will play an integral role both inside and outside the jail. They will work closely with a multidisciplinary team consisting of jail administrators and staff, medical providers, program staff (counselors, re-entry specialists), and community partners such as CSB and others.

This position is full-time and will manage the coordination of services for MAT participants, pre-release, and post-release. This position will be vital to the follow-up support after release and will be the main point of contact for local probation offices, community service boards, mental health/other service providers, re-entry, and peer recovery support specialists (PRSS). Case management will be supported using Lauris Online. This system is designed to meet the needs of clinicians, counseling professionals, and re-entry specialists.

Will manage client cases from intake to discharge. This entails monitoring client progress for treatment plan objectives, assisting with post-discharge planning, monitoring compliance, and assisting with scheduling post-release treatment needs.

Essential Functions

Essential functions include, but are not limited to:

  • Demonstrate the ability to engage individuals in a hopeful, empathetic manner regardless of the phase of recovery
  • Provide a welcoming environment while conducting interviews/screenings for individuals seeking enrollment in MAT.
  • Make referrals to medical personnel who will determine participants' eligibility for MAT.
  • Manage caseload estimated at 10 or 15 participants, with most needing intensive case management.
  • Prepare statistical reports.
  • Access Medicaid portal to determine participants' eligibility.
  • Provide linkage, monitoring, coordination, and procurement of rehabilitation counseling, residential and housing support, vocational readiness and placement support, crisis stabilization, and hospital/jail diversion services.
  • Provide comprehensive assessments for persons involved with the criminal justice system to determine service, supports, and other needs to assist the individual to remain in the community.
  • Develop and implement home plans that reflect the identified needs of participants.
  • For incarcerated participants, conduct follow up with re-entry to ensure appropriate placement in re-entry classes/programs designed to assist participants with reintegration into the community.
  • Coordinates services and service planning with medical providers within NN-MAT and with other agencies such as criminal justice agencies, probation officers, Veterans Affairs staff, jail staff, psychiatric hospitals, counselors, landlords, vocational programs, doctors, etc., to ensure continuity of care.
  • Provides case and program consultation to individuals, organizations, agencies, groups, etc. that are involved with participants.
  • Maintains records related to service delivery according to applicable ethical requirements and the requirements of the Newport News Sheriff's Office and other regulatory agencies (federal, state, and other).
  • Participates in team meetings, professional development training, seminars, and conferences pertaining to NN-MAT and its operation.
  • Provides community integration activities to include assistance with psychiatric and medical appointments, housing stabilization, accessing community resources and services, benefits acquisition, and adherence to legal requirements, and other duties as assigned. Education/Experience

    Bachelor's degree in a human services field.

    Two years' experience in a human services field (i.e., Psychology, Sociology, Social Work, Counseling, Public Health).

    Two or more years of experience working with adults with mental health/substance abuse disorders; preferably in jail-based and community-based programs. One or more years' experience working with criminal justice agencies.

    Prefer Master's Degree in a human services field (i.e. Psychology, Sociology, Social Work, Counseling, Public Health).

    QMHP-A - Registration

    CSAC - Certification

    Experience supervising subordinates in a team environment

    Additional Information & Requirements
    • Possession of a valid driver's license and an acceptable driving record by the DMV
    • Must be certified or certifiable in basic first aid and CPR. Work Location: Newport News City Jail

      Work Schedule: Flexible schedule Monday thru Friday, evenings and weekends as needed.
