Sentara Careplex Hospital located in Hampton, VA is currently
recruiting for an Inpatient Case Manager- RN .
Full-time: 40 hours
Shift: Days
Requirements:
Minimum 1 Year - Case Management, Acute Nurse, Nursing
Education: - RN: Associate's, Bachelor, RN-Diploma and Master
Degree
Inpatient Case Manager, RN - Responsible and accountable for the
provision and facilitation of comprehensive care management,
services, and quality outcomes for patients across the healthcare
continuum, primary focus on the patient transition to the
appropriate level of care while facilitating clinical
appropriateness and LOS. Directly support those in-hospital
patients identified in need of comprehensive case management.
Promotes effective utilization and monitoring of health services,
collaborates and communicates with the healthcare team and
patient/caregiver to manage care and transitions. Develops and/or
implements a comprehensive care plan based on assessment and
evaluation to include the physical, mental, and psychosocial needs
of the inpatient/caregiver. Functions in one of the following
practice settings: Hospitals Only.BLS within 90 days of hire.
Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification
is required within one year of eligibility.
1-year acute care case management experience preferred.

Job Summary

BSN is preferred. All Registered Nurses who do not have their BSN
will be required to sign a BSN Agreement committing to successfully
obtaining their BSN within 5 years of hire.
Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification
is required within one year of eligibility.
1-year acute care case management experience preferred.
Qualifications:
N-2YR - RN-Associate's Degree (Required), N-3YR - RN-Diploma
(Non-degree) (Required), N-4YR - RN-Bachelor's Level Degree
(Required), N-6YR - RN-Master's Level Degree (Required)
Basic Life Support (BLS) - Certification - American Heart
Association (AHA) RQI, Registered Nurse (RN) Single State - Nursing
License - North Carolina, Registered Nurse (RN) Single State -
Nursing License - Virginia, Registered Nurse License (RN) - Nursing
License - Compact/Multi-State License
