community! Responsibilities About the Role During visits that may
last up to one hour, our providers assess and observe a member’s
health status, medical history, medication adherence, social
circumstances, and other potential risks. This approach provides
invaluable insights into a member’s overall health that are often
missed during standard office visits. The Matrix Comprehensive
Health Assessment (CHA) aims to enhance care quality and address
multiple care gaps in a single appointment. Our providers are
responsible for: Conducting assessments for adults and geriatrics,
including medical history, diagnosis, treatment, health education,
referrals to physicians and case management, follow-up, and
thorough documentation in line with our guidelines. Administering
and ordering necessary point-of-care and lab-based tests.
Collaborating with physicians, case managers, social workers,
family members, key caregivers, and other medical personnel as
needed. Working alongside Primary Care Physicians (PCPs) to educate
patients and ensure appropriate follow-up. Providing services at
the member’s residence, whether in their homes or in Skilled
Nursing Facilities. Benefits of Working with Us Flexibility – You
can schedule visits according to your availability. Assessments are
conducted from Sunday to Saturday, between 8 am and 7 pm, allowing
you to work during the day, evening, or weekend as it suits you.
Competitive Pay – Your earnings are based on the number of
assessments completed. By increasing your availability and
traveling to see members outside your home area, you can enhance
your earning potential. Those averaging 20 to 100 visits monthly
can earn between $1,800 and $11,500 , based on a per-assessment
rate of $90 to $115 , depending on your state. Benefits – Includes
mileage reimbursement, a 401(k) plan, employee referral bonuses,
and limited voluntary benefits. Education and Support – We are
committed to your growth! Along with comprehensive onboarding
training, you will be paired with a preceptor, receive personalized
feedback, and gain free access to continuing education
opportunities. Additional Licensure – If there is a business need,
we will assist you in obtaining additional state licensure and
credentialing in neighboring states or others you may wish to visit
while conducting health assessments. Full-time Opportunities – Our
PRN providers have the option to transition to full-time roles
supporting their home location or a specific state or region, along
with a full-time sign-on bonus. Qualifications Skills & Experience
That Shine A Master’s Degree or equivalent experience, along with
successful completion of NP or PA licensure. Current NP or PA
licensure in the state of practice, including prescription
authority or the ability to obtain it. Board certification from
NCCPA, AANP, ANCC, or AACCN in Adult, Adult Gerontology Primary
Care, Adult Gerontology Acute Care, Family, Emergency, or Acute
Care specialties. Current BLS, ACLS, or CPR certification.
Preferred one year of experience as a Nurse Practitioner or
Physician Assistant; however, new graduates are encouraged to
apply! A valid state driver’s license and proof of adequate
automobile insurance for your state of residence. Strong computer
skills and familiarity with health/medical record software.
