positively to the community! Responsibilities About the Role During
visits that may last up to one hour, our providers assess and
observe a member’s health status, medical history, medication
compliance, social circumstances, and other potential risks. This
approach provides invaluable insights into a member’s overall
health that are often missed during standard office visits. The
Matrix Comprehensive Health Assessment (CHA) aims to enhance care
quality and allows us to address multiple care gaps in a single
appointment. Our providers are responsible for: Conducting
assessments for adults and geriatrics, which include reviewing
medical history, diagnosing and treating conditions, providing
health education, making physician referrals, and ensuring thorough
documentation in line with our guidelines. Administering and
ordering necessary point-of-care and lab tests. Collaborating with
physicians, case managers, social workers, family members, key
caregivers, and other medical personnel as needed. Working
alongside Primary Care Physicians (PCPs) to educate patients and
follow up as required. Delivering services at the member’s
residence, whether in their homes or Skilled Nursing Facilities.
Benefits of Working with Us Consistency – While assessments are
conducted from Sunday to Saturday between 8 am and 7 pm, our
full-time providers typically work Monday to Friday from 8:00 am to
5:30 pm. Competitive Pay – Enjoy an annual base salary ranging from
$95,500 to $158,400 , determined by market analysis and experience.

Additional Licensure – If business needs arise, we will assist you
in obtaining additional state licensure and credentialing in
neighboring states or others you may wish to visit while conducting
health assessments. Qualifications Skills & Experience That Shine A
Master’s Degree or equivalent experience along with successful
completion of NP or PA licensure. Current NP/PA licensure in the
state of practice, including prescription authority or the ability
to obtain it as necessary. Board certification from NCCPA, AANP,
AACN, or ANCC in Adult, Adult Gerontology Primary Care, Adult
Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
Current BLS, ACLS, or CPR certification. Preferred experience of at
least one year as a NP or PA; however, new graduates are encouraged
to apply! Willingness to travel up to 15% of the time (including
overnight travel) to various locations as required. A valid state
driver’s license and proof of adequate automobile insurance for
your state of residence. Proficient computer skills and familiarity
with health/medical record software. Exceptional verbal and written
