Job Description Job Description Salary: $21-$26/hour Position
Summary The Qualified Mental Health Professional is responsible for
providing individual skills training, mentoring, case management
and care coordination services for clients in the Intensive In-Home
Program. These services are primarily provided in the home.
Essential Duties and Responsibilities Manages a caseload of
clients, providing services on an intensive basis and utilizing
short-term, solution focused treatment strategies Clearly
identifies problem areas and needs, strategies, and service
objectives supports in determining and meeting service objectives
and to identify additional resources and supports Prepares written
service plan, incorporating input from clients, involved service
providers and wraparound team members, and clearly identifying
problem Provides psycho-educational service to clients, including
individual, family, and interventions and issues-specific or
educational groups (parenting skills, anger management) Responds to
crisis situations with twenty-four hour a day availability
Documents case activities for both clinical and billing purposes
Prepares progress reports as required by referral source or service
purchaser Participates in Child Specific Team, Family Assessment
and Planning Team, and other interdisciplinary, diagnostic, or
planning meetings Collaborates and coordinates with other case
involved service providers and professionals, such as attorneys,
school personnel, social workers, and probation counselors Links
clients with external programs or services, such as health
services, recreational activities, childcare services, financial
resources, employment resources, AA/NA groups, transportation
resources, and others Attends court hearings when families are
court-involved and presents testimony and treatment recommendations
as requested by attorneys, referral source, the court, or client
Guides, collaborates, and coordinates with follow up services with
various community resources to include mentoring services and case
management services, and ensuring the adequate exchange of
information is provided Meets every week with Clinical Director to
provide clinical guidance, assess dynamics and needs of each case,
determine the appropriateness of therapeutic strategies, and assess
progress of service delivery and planning Works with Clinical
Director to increase effectiveness of therapeutic interventions
Prepares written Discharge Summary and Plan Apprises Clinical
Director of workload, case activity, service barriers, and
procedural problems in staff and one-on-one meetings Required
Licenses/Certifications: QMHP Certification CPR/First Aid TOVA
Physical Demands and Work Environment The physical demands

employee to successfully perform the essential functions of this

performing the duties of this job, there must be an ability to
perform a range of motions, including but not limited to, sitting
for long periods of time, standing, walking, climbing/descending
stairs and driving for periods exceeding one hour. The employee
must occasionally lift and/or move up to 25 pounds. The employee
will be required to complete a tuberculosis screening upon
hire.
