Great work you’ll do here:

As a Hospice LPN you will provide skilled and compassionate nursing care to patients and families in their home or community under the supervision of a Registered Nurse and direction of the Plan of Care. You will be responsible for visiting patients to assist in their end of life care, the documentation of care given, and chart maintenance to meet both regulatory and billing requirements.

  • Provides services in accordance with organizational policies and procedures and regulations that define scope of practice
  • Prepares clinical and progress notes on the relevant and specialized nursing services he/she provides
  • Assists the RN and/or physician in performing specialized duties related to end-of-life care
  • Assists the RN in carrying out the plan of care
  • Prepares equipment and materials for treatment adhering to aseptic technique as required
  • Assists the patient in learning appropriate self-care techniques
  • Communicates changes in patient condition and needed changes to the plan of care to appropriate team members and caregivers
  • Participates in staff and IDT meetings as requested
  • Ensures the maintenance and confidentiality of client records and employee information in accordance with Confidentiality policy
  • Attends nursing home case conferences as requested
  • Assists with record reviews if requested
  • Contributes to the effectiveness of case management and coordination of services through open communication
  • Follows directives of case manager and supports the efforts of case coordination
  • Completes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company

  • Outstanding presentation, writing and communication skills
  • Promotes the agency through positive representation and communication of its services
  • Capable of using professional judgement to delegate selected nursing tasks
  • Strong ability to be the public face of the organization, and serve as an advocate for Home Healthcare, Home Hospice and other service lines
  • Creative thinker who embraces thought leadership
  • Capable of making challenging decisions and willing to take responsibility for them.
  • Graduate from an accredited practical or vocational nursing program.
  • One year of hospice care experience preferred; two years’ experience in any clinical setting with a demonstrated ability to manage a group of patients.
  • Current license, (in good standing) as a LPN/LPN in the states of practice.
  • Current CPR certification
  • Valid driver’s license and automobile insurance
